Financial Agreement for Non-Surgical Cosmetic Procedures and Skincare Services at Remi Aesthetics This financial agreement is between Remi Aesthetics ("Provider") and the undersigned patient ("Patient") for non-surgical cosmetic procedures and skincare services provided by Provider. 

1. Payment Obligation: Patient agrees to pay Provider for all non-surgical cosmetic procedures and skincare services provided by Remi Aesthetics, including any additional fees or charges as set forth in Provider's fee schedule. 

2. Payment Method: Payment may be made by cash, credit card, or other agreed-upon method. Provider will require payment in full on the date of providing services. If client has opted for Aglow memberships, they are to acknowledge the remainder of their bespoke memberships. Failing to do so, will incur legal fees by the client. 

3. Cancellation Policy: Patient agrees to provide at least 24 hours’ notice if they need to cancel or reschedule an appointment. Failure to provide adequate notice may result in a cancellation fee. 

4. Refund Policy: Provider does not offer refunds for non-surgical cosmetic procedures or skincare services. However, Provider may offer credit towards future services at its discretion. 

5. Consent to Treatment: Patient acknowledges that they have been fully informed of the risks and benefits of the non-surgical cosmetic procedures and skincare services they will receive. Patient consents to treatment and assumes all risks associated with the procedures. 

6. Confidentiality: Remi Aesthetics will maintain the confidentiality of Patient's medical records and personal information in accordance with applicable laws and regulations. 

7. Responsibility for Payment: Patient is responsible for payment of all fees and charges related to the non-surgical cosmetic procedures and skincare services provided by Provider, on the day of treatment.   

8. Collection and Legal Fees: In the event that Patient fails to pay for services provided by Remi Aesthetics, Patient agrees to pay all costs and expenses incurred by Remi Aesthetics to collect the debt, including court costs. 

9. Governing Law: This financial agreement shall be governed by and construed in accordance with the laws of the state in which Remi Aesthetics services are provided.
